This new information serves as “evidence” for revising practices to improve outcomes across health care. Based on this evidence, you can formulate a question. In this Discussion, you consider the use of evidence-based practice in your own organization and formulate a question that you will need to answer for your portfolio project.

This is called a PICOT question. You will also investigate strategies for overcoming barriers to implementing evidence-based practice (EBP). To prepare: Consider a recent clinical experience in which you were providing care for a patient. Determine the extent to which the care that you provided was based on evidence and research findings or supported only by your organization’s standard procedures. How do you know if the tasks were based on research?

In my recent clinical experience, I was caring for a postoperative patient requiring wound care management. Initially, my approach was primarily based on standard procedures outlined by the hospital policy, which are often informed by expert consensus rather than current research. To determine whether my actions were evidence-based, I critically appraised the guidelines and literature related to wound care. This process involved consulting recent studies highlighting the most effective dressing materials and infection prevention strategies. Through this process, I recognized that certain practices I employed, such as routine dressing changes at fixed intervals, might not be supported by the latest evidence advocating for more patient-centered, condition-based scheduling.

In considering future clinical questions, I identified a specific area for improvement: the use of antimicrobial dressings for certain wounds. To formulate a precise clinical question, I employed the PICOT framework, which stands for Population, Intervention, Comparison, Outcome, and Time. My PICOT question was: “In postoperative wound patients (P), does the use of antimicrobial dressings (I), compared to standard dressings (C), reduce infection rates (O) within 30 days (T)?” This question guides a focused investigation into the evidence supporting different dressing choices and their impact on patient outcomes.

Implementing EBP can be impeded by various barriers, including organizational resistance, limited resources, and lack of staff education, as discussed in “Adopting Evidence-Based Practice in Clinical Decision Making.” One significant barrier in my organization is resistance to change among staff, often stemming from a lack of familiarity with new research or skepticism regarding its applicability. To overcome this barrier, I propose a multifaceted plan that involves ongoing education sessions, peer-led discussions, and the integration of EBP protocols into standard workflows. Providing staff with access to current research articles, along with training on critical appraisal, can foster a culture receptive to evidence-based changes.

In summary, adopting EBP requires deliberate efforts in questioning current practices, formulating precise clinical questions like PICOT, and addressing organizational barriers proactively. By fostering an environment that values continual learning and evidence application, nurses can significantly improve patient outcomes and advance the quality of care.
