Attached you will find a template that will guide you in your QI project. At this time, we will be addressing the FOCUS part of our FOCUS PDSA cycle on the attached template. We will be completing the PDSA portion at a later time. You will also complete the attached fishbone diagram. You may not have every box filled in; it will depend on the issue you are exploring.

Since you have not yet completed a comprehensive review of the literature, for the "Select an intervention" portion of the FOCUS worksheet, list potential solutions. As you complete your literature review, you will select the main intervention you wish to implement.
