The process of integrating evidence-based practice (EBP) into healthcare settings is critical for enhancing patient outcomes, improving quality of care, and fostering continuous improvement among health professionals. For my Research Utilization Project centered on the issue identified in Week One—namely, the inconsistent management of postoperative pain among surgical patients—I will employ two well-established EBP models: the Iowa Model of Evidence-Based Practice and the Stetler Model of Research Utilization. These models provide systematic approaches to translating research findings into practice and will guide my efforts to implement, evaluate, and sustain improvements in postoperative pain management protocols.

The Iowa Model of Evidence-Based Practice, developed by Melnyk and Fineout-Overholt (2015), is a widely adopted framework that emphasizes the systematic identification of clinical problems, the review and appraisal of relevant evidence, and the implementation of evidence-based interventions. In my project, I will use the Iowa Model to systematically assess the current postoperative pain management practices within the surgical unit. This involves initiating a problem-focused trigger—namely, the high incidence of poorly controlled postoperative pain reported in patient feedback. The model encourages the formation of a team to review current evidence, identify gaps between current practices and best evidence, and develop practice changes accordingly.

Specifically, I will utilize the Iowa Model to guide the search for the latest research on multimodal pain management strategies, including pharmacologic and non-pharmacologic interventions. After critically appraising the evidence, I will advocate for practice change by collaborating with the surgical team and nursing staff, ensuring that new protocols are tailored to the specific needs of the patient population. The Iowa Model’s cyclical process promotes ongoing evaluation and refinement, which will support the sustainability of the pain management improvements and foster a culture of evidence-based practice within the unit.

Conversely, the Stetler Model of Research Utilization (Stetler, 2017) offers a structured approach that emphasizes critical thinking, contextual assessment, and systematic planning for the integration of research into practice. I will use this model to facilitate the translation of the evidence gathered through the Iowa Model into actual clinical changes. The Stetler Model guides practitioners through five stages: preparation, validation, comparative evaluation/decision-making, translation/application, and evaluation.

During the preparation stage, I will assess the organization’s readiness for change and identify key stakeholders, such as anesthesiologists, surgeons, and nursing staff. The validation stage involves critically appraising the evidence for quality, relevance, and applicability to our specific clinical setting. In the comparative evaluation/decision-making stage, I will analyze the potential benefits, barriers, and resources necessary to implement new pain management protocols.

The translation/application stage will focus on developing practical guidelines and education for staff, while the final evaluation stage involves monitoring patient outcomes, staff adherence, and patient satisfaction to determine the effectiveness of the implemented practices. The systematic, stepwise nature of the Stetler Model ensures that each phase is thoroughly evaluated and adjusted as needed, leading to sustainable change.

Together, these models will complement each other in my Research Utilization Project. The Iowa Model provides a comprehensive pathway from identifying a problem to implementing evidence-based change, emphasizing organizational and team involvement. Meanwhile, the Stetler Model offers a detailed approach to critically appraising evidence and managing change at the individual and unit level. By integrating these models, I aim to establish a robust framework that facilitates effective translation of research into practice, resulting in improved postoperative pain management outcomes.
