Conversely, a closed system would be self-contained, with limited external interactions, potentially leading to rigidity and resistance to change. Given the current global healthcare crisis marked by rapid changes and unpredictable challenges, the ability to adapt quickly is vital. An open system that embraces complex adaptive system (CAS) thinking is better suited to navigate these complexities.

Complex adaptive systems are frameworks that recognize the healthcare environment as a network of interconnected agents, such as clinicians, administrators, patients, and technology, all interacting in non-linear and unpredictable ways (Plsek & Greenhalgh, 2001). Applying CAS principles encourages flexibility, decentralization of decision-making, and learning from experience, which are essential qualities in responding to crises like pandemics, resource shortages, and evolving patient needs. An open facility adopting CAS strategies enhances resilience and capacity for innovation, enabling healthcare providers to shift processes and procedures swiftly in response to emerging challenges (Holland, 1996).

In contrast, a closed facility might risk stagnation and may be less capable of responding effectively to external pressures and changes. For example, during the COVID-19 pandemic, healthcare institutions with open, flexible systems could adapt protocols, reallocate resources, and implement new safety measures more efficiently than rigid, closed systems (Braithwaite et al., 2020). From the perspective of patient care, openness promotes better outcomes by fostering innovation and rapid problem-solving. For staff, an adaptive environment offers opportunities for ongoing learning, engagement, and collaboration, which are key to maintaining morale and quality of care amid rapid changes.

Implementing complex adaptive system principles within an open framework also encourages a culture of continuous improvement and resilience. It supports decentralized decision-making, empowering frontline staff to respond to situations proactively rather than waiting for top-down directives. This flexibility is crucial in managing daily changes and uncertainties inherent in modern healthcare settings (Senge, 2006). Furthermore, an open system that embraces complexity can foster integrated, patient-centered care models that improve communication, coordination, and satisfaction among patients and staff.

While there are benefits to both models, evidence suggests that the most effective approach during a healthcare crisis is an open system that adopts complex adaptive thinking. It aligns with the need for agility, innovation, and decentralized leadership essential for navigating the unpredictable terrain of global health emergencies. The ongoing learning and adaptability promoted by this approach ultimately lead to better patient outcomes and more resilient healthcare staff. Therefore, striving toward openness and adaptive capacity should be prioritized to meet the demands of current and future healthcare challenges.
