Self-Assessments in this course are designed to promote personal reflection on health habits and behavior change. For each chapter, students will complete a self-assessment that involves applying the Transtheoretical Model (Stages of Change) to their own behaviors. Each assessment must include answers to four core questions: 1) Summary of findings from doing the assessment; 2) The targeted behavior for change; 3) The stage of behavior change the student is in, with explanation; 4) The long-term goal associated with the targeted behavior. Responses should be at least 50 words and demonstrate honest self-evaluation. The assessments help foster awareness of health choices and support planning for behavior modification, emphasizing the importance of understanding one's readiness to change and developing concrete strategies for improvement.

Completing the self-assessment for this chapter provided valuable insights into my current health behaviors and my readiness to make changes. I realized that I tend to respond inconsistently to certain health habits, often showing responses like "sometimes" or "rarely," which indicates inconsistency. This pattern highlighted the need to focus on developing more sustained positive behaviors. Specifically, I identified my target behavior as reducing my intake of processed foods and increasing consumption of whole, nutritious foods. Recognizing this helps me understand where I need to direct my efforts.

Regarding my stage of behavior change, I believe I am in the contemplation stage. I recognize that my current eating habits are not optimal and understand the benefits of healthier eating. However, I still feel somewhat ambivalent about fully committing to significant dietary changes. I plan to advance to the preparation stage by setting specific goals, such as grocery shopping with nutritious intent and gradually replacing processed snacks with healthier options. My plan includes tracking my food intake and gradually increasing my intake of fruits and vegetables over the next month to facilitate this transition.

The long-term goal I am setting to address this behavior is to maintain a consistent pattern of healthy eating by consuming at least five servings of fruits and vegetables daily and reducing processed foods by 50% within three months. Achieving this goal will significantly contribute to my overall health by improving my energy levels, supporting weight management, and reducing the risk of chronic diseases. This goal is realistic, measurable, and directly aligned with my target behavior and current stage of change, providing a clear pathway toward ongoing health improvement.
