Thinking about your practice, what is the question in your clinical setting that you would most like to have an answer to? Please consult with your supervisor or colleagues to determine what the agency considers most important. Develop and share a clinical problem or question from your practice that needs investigation or clarification. Your post should be concise, between 75-150 words, and should highlight why this problem is significant. Use relevant studies or examples to support the importance of this question, and describe the current practice related to it. In my clinical practice as a community health nurse, one prominent issue is the high rate of medication non-adherence among elderly patients with chronic illnesses such as hypertension and diabetes. The question I seek to answer is: "What interventions are most effective in improving medication adherence in elderly patients within our community setting?" This question is critical because non-adherence contributes to poor disease management, increased hospitalizations, and higher healthcare costs. Currently, the practice involves providing verbal instructions and follow-up calls, but adherence rates remain suboptimal (Kheir et al., 2020). Improving adherence could be achieved through tailored educational programs, medication management tools, or caregiver involvement, which are areas requiring further investigation (Civeira-Presson et al., 2018). Identifying the most impactful intervention can inform practice improvements and enhance patient outcomes. This problem was identified through repeated hospitalization data and patient feedback indicating difficulties in medication management, underscoring the need for effective strategies to support adherence and ensure safety.
Engaging my classmates, I notice that some colleagues are also interested in patient safety and quality improvement. I look forward to exchanging ideas and learning from their perspectives on similar clinical questions, which can enrich our collective understanding of effective interventions and contribute to evidence-based practice in diverse settings.
