I have to fill out this form in order to be accepted the field that I chose for my Capstone project class. I chose Westchester General Hospital as my experience field, and my preceptor will be Patricia Garcet (Case manager's director). The questions are as follows:

The Westchester General Hospital has been my chosen field for the Capstone project in my nursing program due to its comprehensive healthcare services and focus on community health. To begin with, I have initiated contact with the administration and have set up an appointment with my preceptor, Patricia Garcet, who serves as the Case Manager's Director at the hospital. During our initial meeting, we discussed the various learning opportunities available, which I will detail in the responses to the assigned questions.
In regards to the first question, I have already made several important contacts with the field experience site.
