Select one of the options (A–E) described below. Choose one option, follow the instructions in the appropriate attachment, then prepare a 2-page (double-spaced) paper that includes each of the bulleted elements listed below. For whichever option you choose, be sure to: Discuss why you chose the project (a paragraph or two). Present a summary of the results/findings (approx. 1 page). The summary of results/findings should be specific to the option (A–E) that you chose. Your reactions to the experience (about half of a page). What you gained from the experience (a paragraph). Provide substance, depth, and application of what has been learned in this class; in other words, try to use information gained in this class to your paper as much as possible.

Each option focuses on different aspects such as case management, family health history, personal knowledge of older relatives, long-term care facilities, or interviews with seniors. The purpose is to demonstrate understanding of aging-related issues, resources, and personal reflections, integrating class knowledge with real-world applications.
Option A: Case Management Study
Identify an elder with some functional dependency or limitations. Describe their situation, including age, housing, family dynamics, physical or mental limitations, and current resources like family and services. Develop a plan to help this individual remain independent within the community, utilizing formal and informal support services. Specify agencies and services—providing contact details such as phone numbers, addresses, and contacts when possible. Summarize current conditions and detail available services to meet identified needs. Highlight specific agencies that can assist and describe the type of help they offer.
Option B: Do You Know Your Parents (or Grandparents) as People?
Complete a questionnaire predicting your parents’ or grandparents’ responses, then have them answer the same questions. Conduct this in person if possible. Use their responses to summarize insights gained, including quotes or examples. Reflect on what this revealed about their experiences, values, or personality, rather than merely listing answers. Include the completed form with your paper.
Option C: Family Health History
Complete provided forms to gather health information from your extended family, at least two generations back (parents and grandparents). Summarize in your paper any health issues showing family tendencies, and discuss proactive steps to prevent or manage these. Reflect on your current health risks based on your habits, addressing how lifestyle choices influence future health and chronic disease risk.
Option D: Long Term Care Facility Visit
Visit a long-term care facility—such as assisted living or supportive housing. Interview a staff member about the facility’s history, mission, organizational structure, routines, funding sources, and staff qualifications. Tour the facility, viewing common areas, resident rooms, grounds, and activities. Use the designated form to document your observations and insights. This experience should provide a practical understanding of elder care environments.
Option E: Timeline Interview
Spend about an hour interviewing an American over 65 about their life experiences and perceptions of historical events. Focus on understanding how their generation perceives and was affected by major societal changes. Try to understand what was unique to this generation’s experiences, shaping their worldview. Avoid simply listing questions and answers; instead, aim for a narrative that captures their personal perspective and the broader historical context.
