For this assignment, I selected "the effectiveness of telehealth interventions in managing chronic hypertension" as my clinical issue of interest. With the increasing prevalence of chronic diseases and advancements in digital healthcare, understanding how telehealth can influence blood pressure management is crucial for designing effective interventions and informing policy decisions.
Initially, my database searches yielded a large number of articles—approximately 1,200—when using broad keywords such as "telehealth" AND "hypertension." These results provided a wide overview of available research but lacked specificity regarding study designs, populations, or outcomes. To refine the search and obtain higher quality evidence, I incorporated additional search terms such as "randomized controlled trial," "adults," and "blood pressure control," using Boolean operators ('AND', 'OR'). This narrowing reduced the number of articles to approximately 350, allowing me to focus on higher-quality, specific studies pertinent to my clinical question.
The initial broad search provided a substantial pool of articles but was too unwieldy for an in-depth review. By adding specific keywords and applying filters—such as restricting to peer-reviewed articles published within the last five years—I improved relevancy and reduced noise in search results. For example, combining "telehealth" AND "hypertension" AND "RCT" significantly decreased the number of results, making the review process more manageable and efficient.
To further increase the rigor and effectiveness of database searches for my PICO(T) question, I plan to employ several strategies. First, I will utilize controlled vocabulary, such as Medical Subject Headings (MeSH) terms on PubMed, which enhances search precision by focusing on standardized indexing terms. For instance, searching with MeSH terms like "Hypertension" and "Telemedicine" will retrieve more relevant studies compared to free-text searches alone.
Second, I will use advanced search features, such as truncation and proximity operators, to capture variations of key terms. For example, using "hypertens*" will include "hypertension," "hypertensive," and related terms. Additionally, proximity operators like "adj" or "next" enable the search engine to find words within a certain distance of each other, enhancing contextual relevance.
Third, applying filters and limiters strategically is vital. Limiting searches to randomized controlled trials, systematic reviews, and meta-analyses increases the quality and relevance of evidence. Also, restricting publication years helps to include only the most recent and pertinent findings, aligning with current practices.
Finally, a systematic approach involves iterative searching: continuously refining search terms, reviewing initial results qualitatively, and adjusting keywords accordingly. For example, if initial searches omit relevant studies, I could brainstorm alternative keywords such as "digital health" or "remote monitoring" that may yield additional valuable evidence.
Overall, these strategies—using controlled vocabulary, employing precise Boolean operators, applying filters, and iterative refinement—will enhance the rigor of the research process, ensuring that my evidence base is both comprehensive and relevant for making informed clinical decisions regarding telehealth interventions in chronic hypertension management.
