The clinical issue of interest I have chosen for this course is the management of chronic hypertension in elderly patients. Hypertension remains a prevalent condition among the elderly, leading to increased risks of stroke, heart attack, and renal failure (Walden University, 2022). Proper management of this condition is crucial to reducing morbidity and mortality. The significance of this clinical issue is underscored by the need to balance blood pressure control with the risk of adverse events such as falls and hypotension, particularly in the elderly population. My focus is on understanding how different interventions, such as medication adjustments and lifestyle modifications, impact blood pressure control and patient outcomes in this demographic.

In conducting initial search results, I used two databases: CINAHL and PubMed. Starting with broad keywords such as “hypertension elderly,” my search yielded approximately 150 articles in CINAHL and 200 in PubMed. As I added specific elements—such as “intervention,” “medication management,” and “lifestyle modifications”—using Boolean operators (“AND,” “OR”)—the number of articles decreased. For instance, adding “intervention” reduced the CINAHL results to 75 articles, and combining all terms (“hypertension AND elderly AND intervention”) further narrowed the results to 30 articles. Similarly, PubMed search results shrank from 200 to 60 articles when applying more specific search terms.

To improve the rigor and effectiveness of my future database searches for my PICO(T) question, I plan to implement several strategies. First, I will use filters such as peer-reviewed journals, publication date within the last five years, and language (English) to increase the relevance and quality of retrieved articles. Second, I will employ Medical Subject Headings (MeSH) terms in PubMed to refine searches further, as MeSH terms help identify articles with standardized indexing, making searches more precise (National Library of Medicine, 2021). For example, I will use MeSH terms like “Hypertension” and “Aged.” Third, I will utilize the Boolean operators strategically, combining broad and narrow concepts to keep a manageable number of articles while ensuring comprehensiveness. Additionally, I aim to incorporate grey literature and clinical guidelines from reputable sources such as the American Heart Association to enrich the evidence base. These strategies will help in gathering high-quality, relevant evidence to inform clinical decision-making effectively.
