Understanding social determinants of health (SDOH) and health equity is fundamental to providing comprehensive and equitable nursing care. The World Health Organization (WHO) describes determinants of health as a complex interplay of social, economic, personal, and environmental factors that influence individual and population health. These factors include the conditions into which people are born, grow, live, work, and age. The five key domains delineating SDOH are economic stability, education access and quality, healthcare access and quality, neighborhood and built environment, and social and community context (WHO, 2021). Recent research emphasizes how disparities within these domains contribute to health inequities, which are unfair, systemic differences in health outcomes among different populations (Bleich et al., 2020). Gaining knowledge about these determinants enhances nurses’ understanding of the root causes of health disparities and informs targeted interventions to promote health equity.
From a theoretical perspective, the Social Ecological Model offers a useful framework for understanding how various levels of influence—from individual to policy—affect health outcomes and disparities (McLeroy et al., 1988). Nurses, positioned at the frontline of healthcare delivery, can address SDOH by advocating for policies that reduce barriers to healthcare, promoting health literacy, and collaborating with community organizations to improve social and environmental conditions. For example, nurses can advocate for equitable access to healthcare services in underserved neighborhoods or participate in community health initiatives that address food insecurity and housing instability. Strategies such as screening for social needs, culturally competent care, and health education are vital tools. Addressing social determinants directly impacts health inequities, fostering conditions where all individuals have the opportunity to attain optimal health, regardless of socioeconomic status (Williams et al., 2019). Ultimately, integrating SDOH awareness into nursing practice aligns with the ethical principles of beneficence and justice, ensuring that care is equitable, personalized, and responsive to societal influences.
