Briefly introduce a person you have recently cared for in your nursing practice. (Be sure not to include any identifying information that would be protected by HIPAA!) Discuss the person’s view of the cause of their health condition, the person’s health literacy, and the person’s identity on the continuum of privilege-disadvantage (Table 4 in the Lor article on p. 361). In recent nursing practice, I cared for a middle-aged male patient recovering from a chronic respiratory condition. To respect privacy, I will refer to him as "the patient." The patient believed that his respiratory illness was primarily caused by environmental factors, such as exposure to pollution and occupational hazards, rather than recognizing potential genetic or lifestyle contributors. His perception of the health condition shaped his preventive behaviors and willingness to engage in certain interventions, believing external factors were beyond his control.

The patient's health literacy appeared limited; he struggled to comprehend complex medical terminology and instructions regarding medication adherence and lifestyle modifications. When explaining his treatment plan, I observed frequent misunderstandings and confusion, which impeded his ability to actively participate in his care. To address this, I employed simplified language, visual aids, and checked for understanding via teach-back methods, thus enhancing his health literacy and empowering him to participate more effectively in managing his health.

Regarding the patient's position on the privilege-disadvantage continuum, he exhibited characteristics leaning toward disadvantage. He had limited access to healthcare resources due to socioeconomic constraints, lacked health insurance, and had low educational attainment. These factors created barriers to optimal health outcomes and contributed to health disparities, indicating a need for culturally sensitive and accessible care strategies.

In delivering culturally competent care, I focused on building rapport through respectful communication and understanding his socio-economic context. I acknowledged his beliefs about environmental causes and validated his concerns, which fostered trust. I also aligned health education with his level of understanding, using culturally appropriate materials and respecting his preferences and values. Collaborating with community resources, such as social services and environmental health agencies, helped address broader social determinants affecting his health. This holistic, patient-centered approach aimed to empower him within his socio-cultural context, ultimately promoting better adherence and health outcomes.

Culturally competent nursing care requires ongoing self-awareness, understanding of diverse cultural backgrounds, and adaptation of practices to meet individual patient needs. By recognizing the influence of social determinants of health and respecting patient perspectives, nurses can facilitate equitable care that improves health literacy, engagement, and outcomes for disadvantaged populations.
